Contractor Survey Form – COOCVE

Download the Contractor Survey and save it as BuildingNameContractorSurvey.
Fill in your responses in the blue fields.
- Make sure to provide your building name and an email contact.
- Use the drop-down menu to select the contractor type. Select ‘Other’ to type in a professional service that is not listed.
- Please provide comments and names of specific contractors, when possible. Both ‘good’ and ‘not good’ ratings are equally helpful for other buildings.
Once you have filled in the data for your last contractor (up to 10), save your completed survey form as BuildingNameContractorSurvey.
Email the survey to [email protected], or print it and drop it by the COOCVE Office.
